Information for mentors-to-be

Buckeye leafThank you for your interest in LASER’s mentoring program. Since you will be working with students under the age of 18, per Ohio State University policies, you will need to complete some very easy and quick paperwork.

A. Background Checks

LASER is required to conduct a background check (free of charge).

To be able to properly conduct the background checks, please bring the following:

  1. Government Issued ID (any of the following are acceptable):
    • Driver's license
    • Passport
    • U.S. military ID (active duty or retired military and their dependents, and DoD civilians)
    • Permanent resident card
  2. Social Security Number: LASER will only need the number and do not require a copy of your social security card.
  3. Background Check Policy 4.15 Fingerprint Consent Form. Please fill out, print and bring with you.*

*NOTE: Fingerprinting services are available Tuesday through Friday, 9-11 a.m. and 1-4 p.m. Fingerprinting services are not available on Monday.

B. Activities and Programs with Minor Participants

Ohio State University policies require students to read and fill out paperwork for employees and volunteers who will be working with minor participants.

Prior to starting the mentorship, please read and sign the Activities and Programs with Minor Participants Office of Human Resources Policy 1.5 0 - Non-Care Custody Control Training.

C. Standards of Behavior form

 Prior to starting the mentorship, please read and sign the Activities and Programs with Minor Participants Office of Human Resources Policy 1.50 Standards of Behavior for Employees/Volunteers.
